Insurance. I provide all the necessary paperwork for my clients to be reimbursed by their insurance provider. The majority of my clients receive some form of reimbursement from their insurance. Please call your insurance provider to determine your coverage.
Sliding Scale Fee. Sliding Scale fees must be preapproved. Please inform me as soon as possible because I do have a limited sliding scale or I can connect you with another therapist to serve your needs.
Cancellation Policy. If for any reason you are unable to keep your appointment, please contact the office to cancel no later than 24 hours in advance. If you fail to notify the office to cancel your appointment, you will be required to pay the full cost of the session.
§ Reproduction of Records (a minimum of 14 business day turnaround for reproduction and delivery of documents). There is a $25 fee for all transmission of requested documents. All incurred delivery fees (if applicable) will be the responsibility of the requesting party.
